Premium Home Health is a home health agency located in Norwalk, CA. We currently provide home health services to patients within a 50 mile radius in any given direction from our offices in Norwalk.

We are currently seeking qualified candidates to fill the following field positions, please see the job descriptions below:

- RN (Orange County and LA, Spanish speaking preferred)

- LVN (Orange County and LA, Spanish speaking preferred)


- PT (Orange County and LA, Spanish speaking preferred)

RN:

RN responsibilities include but are not limited to the following:

- Completes initial, comprehensive and ongoing assessments for patient and family

- Develops care plan in accordance with patient and family needs

- Provides nursing care according to nursing process and care plan

- Supervises medication administration and regimen

- Documents and submits patient progress/status in a timely manner

- Provides teachings relevant to patient condition and diagnosis

- Assists in overall care of the patient

- Complies with Premium Home Health policies and procedures and maintains confidentiality and safety of patient records so that no information is divulged to other than proper users.

- Must be able to lift, position, and transfer patient. Must be able to lift supplies and equipment. Capable of prolonged or considerable walking/standing, capable of reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing required to perform necessary nursing skills.

- One year of nursing experience required.

LVN:

LVN responsibilities include but are not limited to the following:

- Provides patient care according to comprehensive assessment and plan of care

- Provides accurate and timely documentation of patient services to reflect plan of care

- Provides teachings relevant to patient condition and diagnosis

- Provides appropriate pain and symptom management and evaluates patient’s response to treatments and medication

- Performs duties assigned by RN

- Assists in overall care of the patient

- Complies with Premium Home Health policies and procedures and maintains confidentiality and safety of patient records so that no information is divulged to other than proper users.

- Must be able to lift, position, and transfer patient. Must be able to lift supplies and equipment. Capable of prolonged or considerable walking/standing, capable of reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing required to perform necessary nursing skills.

- One year of nursing experience required.

PT:

PT responsibilities include but are not limited to the following:

- Provides physical therapy services to patients according to a written individualized plan of care.

- Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.

- Assesses for muscle strength, mobility, gait, ROM and transfer capabilities.

- Directing physical therapy treatment.

- Instructing patients and families/caregivers in the use and care of therapeutic appliances.

- Determining priority needs for physical therapy

- Reporting to physician patient's reaction to treatment or changes in condition.

- Initiates physical therapy program and instructs other personnel and/or family/ caregivers in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participat ing in case conferences.

- May educate patient in the use of prosthetic device.

- Prepares and submits clinical and progress summaries based on the attainment of goals.

- Participates in patient discharge planning. Prepares and submits a clinical progress summary based on the attainment of goals directed by organization policy.

- Provides physical therapy consultation to families/caregivers, as appropriate.

- Must possesses a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization. Meets personnel qualifications stated in §418.114(b)(7).

- Licensed to practice as physical therapist within the state.

- At least two years of appropriate experience as a physical therapist.

-Demonstrates good verbal and written communication, and organization skills.

- Possesses and maintains current CPR certification.

CHHA

Certified Home Health Aide

Essential Job Functions/Responsibilities

Responsibilities of the home health aide include, but are not limited to, the following:

1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly.

2. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.)

3. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned.

4. Assisting in dressing and undressing patients as assigned.

5. Assisting in feeding the patient as assigned.

6. Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency.

7. Reporting on patient's condition and significant changes to the Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues.

8. Assisting in ambulation and exercise as instructed by the home health nurse or therapist.

9. Performing range of motion and other simple procedures as an extensional therapy service as ordered with appropriate completed/demonstrated skills competency.

10. Keeping patient's living area clean and orderly, as assigned.

11. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct.

12. Respectful of patient and family/caregiver environment and patient's personal needs.


Position Qualifications

1. A person who has successfully completed a state established or other training program that meets the requirements of S 484.36(a) and a competency evaluation program, or state licensure program that meets the requirements of S 484.36(b), or a competency evaluation program or state licensure program that meets the requirements of S 484.36(b).

2. At least 18 years of age.

3. Ability to read and follow written instructions and document care given.

4. Satisfactory references from previous employers.

5. Possess and maintains current CPR certification.

6. Demonstrates tact, patience and good personal hygiene.

7. Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.

CASE MANAGER:

REPORTS TO THE RN CLINICAL COORDINATOR

SUMMARY:

The Case Manager is a qualified professional responsible for the coordination and delivery of quality home health care to the patient in accordance to the State and Federal regulations.

QUALIFICATIONS:

1.    Currently licensed though the State Board of Nursing

2.    At least one year clinical experience consistent with the services that are provided by Premium Home Health, Inc.

3.    Must possess good nursing judgment and excellent oral and written communication skills.

4.    Complies with acceptable professional standards and principles.

5.    Capable of working with minimal supervision, resourceful, and with good organizational skills.

6.    Flexible and cooperative in fulfilling role obligations.

ESSENTIAL JOB FUNCTION/RESPONSIBILITIES

1.    Instruct and guide clinicians to promote effective performance and deliver of quality home health care.

2.    Ensure that the services provided are coordinated with the physician and other ancillary personnel.

3.    Ensure that the frequency of visits are being followed as per plan of care.

4.    Coordinate utilization review along with the RN clinical coordinator to ensure that there is no apparent over utilization or underutilization of services.

5.    Generate infection control, patient incident/grievance report as needed.

6.    Perform patient satisfaction surveys on all patients assigned.

7.    Generate telephone orders, communication notes, and transfers under the supervision of the nursing supervisor.

8.    Assume additional responsibility as designated by the Director of Patient Care Services.
